Since October, Google has been testing 2026 ads for selected prescription drugs in its AI search. The company confirmed to Adweek a test limited to the USA in AI Mode and AI Overviews. Standard text ads and shopping ads can appear in areas labelled "Sponsored". Google has not announced a launch for Germany.
What is new – and what is not
What is new is the possible placement of such pharma ads around AI-generated search answers. After an additional query from the industry portal Search Engine Roundtable, Google did not change its existing pharmaceutical advertising policies for this. The test is therefore not a general licence for pharma marketing, nor evidence that ads determine the content sources of an AI answer.
Google had already explained AI-based ad formats in its search marketing programme. According to the company, the current experiment merely extends the product categories included in the test. No public directory of participating companies, concrete reach figures or performance results are available.
Why this does not mean a rollout in Germany
Google's own Google Ads policy allows pharmaceutical manufacturers to advertise prescription medicines only in Canada, New Zealand and the USA, and requires Google certification. Germany is not on this list. For over-the-counter medicines, Google names other approved markets, including Germany. The categories must not be confused; in addition, the respective national advertising law applies.
The combination of a US advertising test and European rules is particularly tricky for agencies. Anyone who promises across the board that "pharma ads in Google's AI answers are now possible" conflates US test access, product approval and geography. That is not a reliable statement for German campaigns.
Visibility and attribution remain two different tasks
Even if an ad appears in the AI search environment, it does not follow that the brand is cited as an organic source or that additional sales result. Marketing teams need separate metrics for ad impressions, qualified traffic and actual conversions. In principle, advertising in AI interfaces is becoming ever more relevant: our analysis of the ChatGPT advertising market shows a different platform model, while our article on AI search and media budgets explains the shifts in budgets.
The distinction also matters for automated campaigns: the evaluation of automated US ad spend does not replace a review of individual campaign results.
Practical consequence: German agencies should only make concrete booking commitments to clients for pharmaceutical AI search ads once Google has confirmed country, product category, certification and the ad placements available for the specific account. Until then, this is a US test without published performance figures.
